چکیده
Hilden lists some of these limitations in his letter; another important one is its global interpretation, which does not translate well into clinical practice (1 ). The partial area under the ROC curve in the clinically relevant range of false-positive rates, or falsenegative rates, is a useful alternative measure of inherent diagnostic accuracy. Dr. Hilden advocates performance measures that incorporate the pretest probability of disease and the clinical benefit and loss of correct and incorrect diagnoses. These measures are extremely useful, but they are not measures of inherent test accuracy. Our recent review (2 ) focused on problems with the analysis of ROC curves, without critiquing issues of study design. I would encourage investigators to carefully consider the objectives of their study and use the most relevant measure of diagnostic test accuracy.

Free or total metanephrines for diagnosis of pheochromocytoma: what is the difference?
Pheochromocytomas, although a rare cause of hypertension, are dangerous tumors that require consideration in large numbers of patients. The resulting low prevalence of these tumors among the tested population and the inadequate sensitivity and specificity of commonly used biochemical tests make diagnosis of pheochromocytoma difficult and time-consuming. As outlined in two articles in this issue...
متن کاملPlasma chromogranin A or urine fractionated metanephrines follow-up testing improves the diagnostic accuracy of plasma fractionated metanephrines for pheochromocytoma.
CONTEXT The initial diagnosis of pheochromocytoma relies on plasma fractionated metanephrines levels. Normal levels exclude pheochromocytoma, but positive tests have a low positive predictive value due to the disease's rarity. OBJECTIVES The objective of the study was to evaluate three approaches to distinguish between true-positive and false-positive tests: 1) increased cutoff for plasma fra...
متن کاملBiochemical diagnosis of pheochromocytoma: which test is best?
CONTEXT Diagnosis of pheochromocytoma depends on biochemical evidence of catecholamine production by the tumor. However, the best test to establish the diagnosis has not been determined. OBJECTIVE To determine the biochemical test or combination of tests that provides the best method for diagnosis of pheochromocytoma. DESIGN, SETTING, AND PARTICIPANTS Multicenter cohort study of patients te...
متن کاملStability of urinary fractionated metanephrines and catecholamines during collection, shipment, and storage of samples.
BACKGROUND Measurements of 24-h fractionated urinary metanephrines and catecholamines are used for the diagnosis of pheochromocytoma, but adequate information is needed regarding collection, storage, and shipment conditions. METHODS Spot urine samples were collected from 8 healthy volunteers. Aliquots were immediately frozen at -20 degrees C, or acidified to pH 4 and then frozen either direct...
متن کاملDifferent diagnostic cut-off values of urinary fractionated metanephrines according to sex for the diagnosis of pheochromocytoma in Korean subjects.
The diagnosis of pheochromocytoma depends on the documentation of catecholamine overproduction. The use of urinary fractionated metanephrines has recently become common for the diagnosis of pheochromocytoma. In order to avoid false positive and false negative results, optimal cut-off levels are necessary; however, there have been few published reports on whether different cut-off levels are n...
